A physician or nurse practitioner completes the referral form and sends supporting documents by fax.

The referral is reviewed, and the patient is contacted for virtual onboarding and appointment scheduling.

The patient meets virtually for a structured medical cannabis assessment and individualized education.

Plan & follow-up

When appropriate, a care plan is provided and follow-up is arranged to monitor progress and tolerability.
