For patients

Medical cannabis care starts with a referral.

Elements Medical Collective is a virtual Ontario clinic providing medical cannabis assessments for referred patients. We help patients understand whether medical cannabis may be appropriate, how to approach treatment safely, and what follow-up is needed.

Accessing care

Please speak with your physician or nurse practitioner.

At this time, Elements Medical Collective accepts referrals from physicians and nurse practitioners. If you are interested in exploring medical cannabis, please ask your clinician whether referral for assessment may be appropriate.

Important: A referral and consultation do not guarantee authorization. Suitability is assessed case by case based on clinical context, risks, and patient goals.

What to expect if you are referred

  • Your referral is reviewed before booking.
  • You are contacted for virtual onboarding and appointment scheduling.
  • Your medical history, symptoms, medications, and goals are reviewed.
  • You receive education and a plan if medical cannabis is appropriate.
  • Follow-up may be arranged to monitor response and side effects.
