Referral process

Complete referral form

Include patient demographics, reason for referral, relevant cautions, and the referring clinician’s contact details.

Attach documents

Please include the cumulative patient profile, medication list, and relevant notes, labs, or imaging if available.

Fax securely

Fax the completed package to 613-344-1203. Once received, the referral is reviewed for next steps.

Booking & documentation

The patient is contacted for virtual onboarding and scheduling, and consultation documentation is sent back after assessment.

Who may be appropriate to refer?

Patients may be referred for assessment when medical cannabis is being considered as part of symptom management and when a structured medical review would be helpful.

  • Chronic non-cancer pain or neuropathic pain
  • Fibromyalgia or complex pain presentations
  • Sleep disturbance related to chronic symptoms
  • Spasticity, oncology, or palliative symptom burden
  • Other complex symptoms where cannabinoid therapy is being considered

Clinical screening considerations

Authorization is not guaranteed. Patients are assessed for suitability, risks, medication interactions, and relevant cautions.

  • Psychosis or bipolar I history
  • Current or past cannabis/substance use disorder
  • Pregnancy, breastfeeding, or planning pregnancy
  • Significant cardiovascular, liver, or kidney disease
  • Allergy to cannabis, cannabinoids, or formulation excipients
  • Younger patients where neurodevelopmental risk is relevant
