Do patients need a referral?

Yes. At this time, Elements Medical Collective accepts referrals from physicians and nurse practitioners.

Are appointments virtual?

Yes. Elements Medical Collective operates as a virtual clinic for eligible patients across Ontario.

Does a referral guarantee medical cannabis authorization?

No. A consultation does not guarantee authorization. Suitability is assessed case by case based on symptoms, medical history, medications, goals of care, and safety considerations.

What documents should be sent with the referral?

If available, please include the cumulative patient profile, current medication list, and relevant consult notes, lab results, or imaging.

Do you sell cannabis products?

No. Elements Medical Collective is a medical clinic and does not operate as a cannabis retailer.

Can patients email medical questions or records?

No. Email is for general administrative questions only. Please do not send personal health information, medical records, or symptom-specific questions by email.

What happens after a referral is received?

The referral is reviewed and the patient is contacted for virtual onboarding and appointment scheduling.

Will the referring clinician receive documentation?

Yes. Consultation documentation is sent back to the referring clinician to support continuity of care.

What if the patient is palliative or urgent?

The referral form includes an expedited/palliative option. Referring clinicians should indicate urgency clearly and include relevant supporting context.

What should patients prepare before the appointment?

Patients should be prepared to discuss their symptoms, current medications, prior cannabis experience if any, allergies, medical history, and goals of care.

Is this an emergency service?

No. This is not an emergency service. Patients experiencing a medical emergency or severe adverse reaction should call 911 or go to the nearest emergency department.
